Modern plastic surgery approaches to facial rejuvenation have changed greatly in the last 15 years. They center around three key procedures that match up to the three zones of the face: endoscopic brow lift for rejuvenation of the upper third of the face; blepharoplasty and mid face lift for the middle third; and lower face and neck lift for the jowls and neck.
Endoscopic techniques make use of operating telescopes to enable complex surgical procedures to be performed through small incisions. This modern technology has been most successful in the upper third of the face where it clearly decreases the length of scars within the scalp when compared to standard open brow lifts. Two small incisions are made behind the hairline directly above the eyebrows and a slightly longer incision is made in each temple region. The endoscope and specially designed tools are inserted through these incisions to release and lift the skin and soft tissues of the forehead. Special scissors are used to divide and weaken the muscles between the eyebrows which are responsible for undesirable frown lines. We use dissolving sutures to hold the forehead scalp in the lifted position until post operative healing keeps it there permanently.
The eyes and cheeks, or mid face, is a very important region in facial rejuvenation. It is the center of the face and comes under close scrutiny when gazing into a persons face. Our philosophy is to rejuvenate this region without “over sculpting” the eyes as was too often seen in traditional plastic surgery procedures. The upper lids can have the excess skin removed by an excision which hides the scar in the crease of the open upper lid and avoids a hollow look by conservative removal of fatty tissue only when necessary. The lower eye lids are operated on through an incision hidden below the lower lid lashes. To obtain the most dramatic results with a natural look the lower blepharoplasty should be combined with a mid face lift. This is performed by continuing the surgical dissection beyond the lower eye lid into the upper cheeks. This maneuver is accomplished quickly and safely by operating under the squinting muscle of the lower lid and into the blanket of connective tissue and fat that gives the cheeks their shape. This layer referred to as the SMAS, by anatomist, can be pulled back up onto the cheek bones and held in position by a single permanent suture. This important maneuver restores a tight youthful cheek region, dramatically improves the shape of the lower lid, and even smoothes the line between the cheek and mouth referred to as the naso-labial fold. The excess skin of the lower lid can then be aggressively removed and the incision sutured closed without danger of pulling down on the lower lid edge - a complication of standard lower blepharoplasty known as ectropian.
The jowls and neckline as well as the skin of the cheeks are best treated with a lower facelift. Many minimally invasive maneuvers have been tried to avoid the scar that is placed in front and behind the ear that is used in this technique, but none have equaled the success seen with this approach. The key to a great result and scar that is not noticeable is to leave the tension of the lift on the deep structures of the face (the SMAS layer and platysma muscle) and minimize tension on the skin. Sometimes liposuction below the chin is performed and this fat can be injected to the naso-labial fold, chin, or jaw line.
These facial procedures are commonly performed as outpatient surgery in our office under a local anesthesia with twilight sedation. All sutures are removed by the first week post op. Patients are allowed to wear cover up make up that our office’s aesthetician applies shortly there after. Most patients return to work and social engagements within two weeks following surgery.
